A report on the Cortico-Basal Implantology Conference that took place at hotel Clarks Amer, Jaipur 2019
The words, Jaipur will not only be called as the Pink City but very soon, will also be called as the City for Dental Implantology, received a thunderous applause by all those who were present for the Inaugural ceremony of the Cortico-Basal Implantology Conference that took place at Hotel Clarks Amer from the 13th-15th December 2019.
The Cortico-Basal Implantology Conference covered every aspect an experienced as well as a budding Implantologist should be aware of, before placing a Basal Implant. The theme, Aesthetic & Prosthetic Advancements in Cortico-Basal Implantology fitted best as per the scheduled pre-conference courses as well as scientific sessions.
The Pre-Conference sessions were held at two locations in Jaipur
The 1st venue was Mahatma Gandhi Dental College and hospital, Sitapura and Basic aspects of Basal Implants – ‘Hands-on’ was the topic of discussion. Dr. Vivek Gaur & Dr. Kiran Patel were the course directors and Dr.Rahul Vaid & Dr. Neelam Maheshwari were the Co-directors. A house full audience with practitioners as well as bright and young students made it a perfect venue for the Pre-Conference session. Topics of discussion ranged from basic aspects to surgical aspects of basal implants followed by a hands-on (STL models).
The 2nd venue had a very special guest, Dr. Antonina Ihde, the head of International Implant Foundation who along with Dr. Anita Doshi were the course directors at Jaipur Dental College. Dr. Prashanth Pillai and Dr. Puneet Bhargava were the Co-directors. Prosthetic aspects of basal implants were the main agenda at this venue and topics of discussion ranged from understanding the prosthetic aspects, demonstration of different prosthetic steps followed by hands-on (STL models).
The evening of the 13th December marked the official inauguration of the conference. Eminent dignitaries graced the opening ceremony with their presence.

To light the lamp is to invoke the presence of the divine. The lightening of the lamp is a traditional culture in India with a promise of a successful beginning to an event.
Dr. Antonina Ihde, Pioneer in developing special implication of occlusion for Basal Implants kick started with an orientation lecture on the topic Need of an Hour. She gave a brief insight on what makes Basal Implantology a unique science in this speed fast world and how patients can go back to their normal life within 3-5 days.
Dr. Vivek Gaur enlightened the audience with his power packed opening lecture on the topic Rehabilitation of Resection Cases.
The aim and objective of his opening lecture was to cover important points such as limiting treatment, enhancing the quality of life and rehabilitation of defect, flapless procedures, buttress and cortical engagement, no dependence on crestal cortical/ alveolar bone, force distribution inline of buttress, Tripodization, Orthopedic/ traumatology principles, smooth principles, smooth surface, bendability, splinting, simple and most accepted/ convenient prosthetic design, No Cantilever etc.
A hi-tea and an Inaugural Dinner was arranged for all the delegates after the opening lectures and thus the 1st day of the Cortico-Basal Implantology conference came to an end.
Time isn’t the main thing, it’s the only thing. Keeping in mind the tight schedule, day 2 of the conference began sharp on time.
It was day 2 of the conference on Cortico-Basal Implantology. Putting up such a huge conference needs day of planning and months of meticulous hard work. Day 2 had a band of erudite strategic Implantologist who covered topics ranging from Introduction & Fundamentals to the Surgical aspects in Strategic Implantology.
Dr. Kapil Kalra conveyed a strong message in the opening Scientific Lecture. He spoke on the topic TECTONIC SHIFT in Implant Dentistry, “Corticobasal implantology” – A solution to every situation.
Key take home points from his presentation Cortico-basal implantology indeed has a solution to every situation. Change is the only constant and Basal Implantology has opened doors to a new and different kind of Implantology and has also emerged as the perfect solution to all the implant related problems in 3-5days. He defined Strategic Immediate loading Implantology as the science involving the judicious application of unique crestal and basal implants with advanced designs in a strategic manner to facilitate their immediate functional loading with the prostheses within a matter of 72 hours From multiple surgeries, multiple incisions, bone grafting, many kits, lot of hassle to just one single kit and minimal discomfort to the patients, this is the Shift that Basal Implantology offers. |
2nd lecture scheduled for the scientific session was on the topic Immediate Loading – Relevant History which every Implantologist should know which was delivered by Dr. Prashanth Pillai, Oral & Maxillofacial Surgeon.
Key take home points from his presentation In his lecture he took the delegates back to many centuries to give them an insight to how immediate loading implantology started. It is extremely important to know the history of immediate loading implantology from a practitioner’s point of view. This gives the practitioner an insight as to how the science has developed over decades right from the pre-historic era to the 19th Century when modern implantology started again with Immediate Loading and how it has been under constant research and development by many French & Italian Dentist. Immediate loading Implantology has taken several centuries of development and now is a completely scientific and evidence based science. |
The extremely energetic Dr. Rahul Vaid gave the next scientific presentation on the topic Bone sets the Clock.
Key take home points from his presentation Nature is supreme and we as individuals need to align with nature to attain optimum results. Similarly, is the 72-hour absence of osteonal activity in these 3 days that prompts us to start and finish the implant case and the loading od the implant in 72 hours and in the right place i.e. in the cortical bone.Thereby bone setting the clock. |
Dr. Nancy Dhiman, during her lecture, spoke on the Disciplines of Strategic Implants.
Key take home points from her presentation For any science to be predictable and reproducible, core principles and essential requisites must be established. These Principles show the anatomic landmarks and the different surgical methods to anchor the cortical implants. She explained the significance of first second and third cortical bone and the participants got a thorough understanding of the applied anatomy and biomechanics involved in the placement of implants in different areas of the mandible and maxilla. |
Dr.Syed Akifuddin spoke on a very crucial topic Informed consent and practice management in Immediate Functional Loading Implantology
Key take home points from his presentation Dr. Syed addressed the delegates on how patient seeking the services of a cortico-basal implantology is to be counselled and prepared. The method and the salient features of an informed consent for a cortico-basal implant procedure was thoroughly discussed. He rightly mentioned the fact that those who are afraid to say NO to their patients should stay away in their own interest from Cortico-Basal Implantology. Dr. Syed gave a clear picture for whom isn’t Cortico-Basal Implantology. And how an individual’s unwillingness to unlearn their conventional implant treatment methods will be of no help keeping in mind the economics. He made a strong point by expressing the fact that Cortico-Basal Implantology is not for individuals who wish to make a quick buck with the use of this technology without investing adequate time and effort in training. Having in depth knowledge of the science is of the utmost importance before taking a crucial step of placing a dental implant in the patient. Dr. Anita Doshi spoke on the topic Myths and Facts of Cortico-basal Implantology
Key take home points from her presentation Myths & Facts are always circulated in any fields. What’s right and what’s wrong depends on the person who experiments it by himself/herself. Replacing all the teeth using dental implants has historically been a challenge due to the lack of bone available for implant placement or proximity to vital structures that need to be avoided (the sinus cavity in the upper jaw and the nerve canal in the lower jaw). Because of which many patients are rejected for implants or they need advanced grafting procedure prior to implant placement. Bone Grafting can be painful, costly and might take several months or longer to heal and yet the results are unpredictable. In the revolutionary approach by placing Bi-Cortical Implants without raising a flap, a permanent full fixed dental bridge can be placed within 72 hours without bone grafting. This is made possible with the use of strategic implants. But there are a lot of myths regarding this newer approach and she covered all of those based on facts during her scientific lecture. Dr. Anita also answered the question of retrievability in this system with multi-unit abutments on strategic implants which offers a powerful, component-based protocol which standardize the necessary angle and level the prosthetic platform. |
Dr. Chandrahas, a periodontist and strategic Implantologist spoke on the topic Predicting the intraoperative events based on radiographic and case history.
Key take home points from his presentation Knowing the history of the patient is of the utmost importance. Medical and systematic examination of the patient aids in the proper treatment. A perfect case history makes the pathway for the final diagnosis, thus making the intraoperative procedures more predictable, Radiographs makes the life of Implantologist very simple and reveals the secret aspect of the available bone. |
Dr. Neelam Maheshwari spoke on yet another important topic in basal Implantology i.e. Importance of Implant Design in Immediate Functional Loading
Key take home points from her presentation For long term success of the implant, selecting the right implant is as good as making the most crucial decision before the surgery. As Strategic Implantologist we are utilising the patient’s assets (available bone). Procedures like sinus lift, bone augmentation and GBR are not required hence saving time as well as cost for the patient. |
After the lunch break Dr. Kiran Patel, a passionate speaker and a Strategic Implantologist delivered a lecture on the topic Practical Guide for beginners.
Key take home points from his presentation Dr. Kiran’s presentation highlighted the steps on how to place a bi-cortical screw implant in different anatomic situations in case of mandible and maxilla in a correct and desired strategic position in accordance to the principles laid down by the International Implant Foundation. Saying no to patients with high expectations is to be aleays kept on mind. The CBCT helps to locate the inferior alveolar nerve in atrophied cases, drilling directions and orientation are strongly recommended for beginners to avoid short preparation and not engaging into the cortical bone. He also gave valuable points to beginners on implant bending, drilling sequence and how to hold the maxilla etc. |
Dr. Murugavel gave a scientific presentation on the topic Anterior mandible surgical anatomy, techniques and complications
Key take home points from his presentation Its seems very easy to place implants in anterior mandible but at times there’s a marble like consistency of bone which gives too much of worry to the clinician. There are at times variation from mental nerve but bone trajectories forming the buttresses in these areas give the best strategic position for placing the implants in anterior mandible and successful prosthesis. |
Dr. Sankalp Mittal gave a brief presentation on the topic Applied Anatomy of the Inferior Alveolar Nerve.
Key take home points from his presentation Must understand the anatomy and surgical part of the Inferior Alveolar Nerve before starting with the planning or placing the dental implant because the inferior alveolar nerve once damaged will be very difficult for a definitive treatment. Spending extra 10 minutes with Radiographs and CBCT’s is highly recommended before proceeding with the procedure. Being very careful will only help the clinician to avoid an failures and damages especially to the inferior alveolar nerve. |
After a short tea break, Dr. Leela Krishna spoke on the topic Tuberopterygoid Implants: The Game Changer
Key take home points from his presentation Tuberopterygoid Implants is one of the important techniques to be learnt for manging maxillary edentulous cases where there is minimal bone thus avoiding secondary surgical procedures like bone grafting & sinus lift. Dr. Rohit Yadav gave a lecture on the topic Nasal Floor Engagement and Canine Bypass

The 2 main objectives of Cortico-Basal Implantology are the use of existing bone of the patient and typical immediate functional loading. Dr. Bobby Antony gave a lecture on the topic Avoiding Sinus Lifts in Cortico-Basal Implantology.
Key take home points from his presentation The presence of the Maxillary Sinus has always been a perennial problem in posterior rehabilitation of the maxilla. Lifting and grafting the Sinus either directly or indirectly had been a procedure of choice to tackle a dipping sinus. Due to the complexity of the procedure, chances of grafts being later infected and most importantly delaying the loading of the implant for more than a year has seen more dentist denying implant rehabilitation of patients citing “lack of bone”. With the advent of Cortico-Basal Implantology, we learn to look beyond the unpredictable maxillary sinus and learn to engage the more predictable bone around it thereby increasing the success rate of our implant rehabilitations and making the need for sinus lifts almost nil. |
Dr. Mehul Jani took the last session of the day 2 of the Cortico-basal Implantology Conference as he spoke on the Topic Guidelines for placement of KOS Implant
Key take home points from his presentation KOS implants are compression screws designed for single-stage procedures. They are alternative offering strong and lasting benefits in quality of life. Immediate and long-term restorations can be implemented safely and with long term predictable success. This treatment protocol is very simple to perform, notably because the bendable single piece implant design of the KOS-micro, and non-bendable KOS-x system permits an immediate functional loading with advantage of initial Osseo-fixation and osseointegration at later stage which will provide anti rotational property to these implants. |
After some mind-blowing Scientific session delivered by the best Master Strategic Implantologist’s it was time for some music and Dance. A grand gala night was organised for all the delegates on Day 2 of the Cortico-Basal Conference.
A melodious Gazal Night got everyone grooving to some very old unforgotten Hindi tracks. The DJ burned the floor with some foot-tapping music. The ambience was favourable for everyone to come a shake a leg. The grand Dinner got everyone together to brainstorm on the scientific lectures as well as to talk about the melodies they all heard.
Day 3 of the Cortico-Basal Implantology Conference
Dr. Antonina began with the opening scientific lecture as she spoke on the Prosthetic aspect of Cortico-Basal Implantology, she spoke on the topic Bridge Framework design on Basal Implants.
Key take home points from her presentation The entire process of rehabilitation of Cortico-Basal Implantology should be completed in 72 hours. In her presentation Dr.Antonina spoke about various frame designs from designing of frames for metal to acrylic bridges and metal to composite bridges, bonding and retention of the acrylic teeth, the shape of the frame for metal ceramic bridge work and other alternative materials and designs that are used for the prosthesis. Her presentation also highlighted the digital workflow in fabricating the prosthesis and showed some slides with awe-inspiring work done by master Implantologist from around the globe and gave a new perspective to all the delegates through her presentation. |
Dr. Faizur Rahamaan spoke on the most important topic the 3 Strategic Principles of Occlusion.
Key take home points from his presentation During his scientific presentation Dr. Faizur Rahamaan discussed what is Occlusion, why occlusion is important and what are the Strategic Principles of Occlusion. He discussed the topic with his vast knowledge on the topic and also discussed as per the guidelines laid down by Ihde & Ihde in their publications. An occlusion can be judged only by the reaction it may or may not have on the teeth or implants along with the bone, tissues and the TMJ. The results on the Occlusal Guidelines are impeccable and when applied judiciously provide the patient with long lasting results. |
Dr. Jugal Patel gave a presentation on the topic Measured Occlusion in Strategic Implantology.
Key take home points from his presentation Occlusion has always been the key to long term dentistry, more so in implant-based rehabilitation. He discussed how with the advent of technology all can precisely measure bite forces, jaw movement, force distribution, muscle activity. This gives clinicians the edge in precise control of forces, helping to balance the longevity of the implants and prosthesis. |
Dr. Mahendra Perumal spoke on the topic Corticobasal Implants in reconstruction of resected and traumatised jaws.
Key take home points from his presentation Cortico-Basal Implantology is a very trending science in the field of oral implantology all around the globe which is setting ablaze in all horizons with its wide vistas of application. Reconstruction of resected jaws, though achieved with favourable results the procedure has always stood to be over enduring, very expensive and highly technically demanding. Through his scientific presentation Dr. Perumal shared his experience of using Cortico-Basal Implant in difficult situations like loss of soft and hard tissue, severe mental trauma, loss of work hours delayed completion of rehabilitation due to need for graft healing osteointegration. Dr. Mahendra also discussed about the less explored arena for Strategic Implantology. |
No one speaks about the failures, But for Dr. Vivek Guar, in the last lecture spoke on the failures. He ended the Scientific Sessions as he gave a presentation on the topic How to avoid Failures in Cortico-Basal Implantology.
Dr. Vivek discussed several failure cases and how he found an alternative to every single one. He made all the delegates aware of the fact that failures should be avoided by paying keen attention and also by examining the patient for extra 10 minutes. To avoid failure, one should have in depth knowledge about that particular subject. For basal implantology one should know the 3- Dimensional anatomy of head and neck and should also have profound knowledge of bone as well. As Cortico-Basal Implantology follows the orthopaedic concepts and well-defined principles, if one keeps in mind all the principles and knowledge together, he/she will never fail and will always have satisfied and happy patients.
This Marked the end of the Scientific Sessions of the Cortico-Basal Implantology.
