Cone Beam Computed Tomography (CBCT) has transformed dentomaxillofacial imaging by overcoming several inherent limitations of conventional two-dimensional radiography, particularly anatomical superimposition, geometric distortion and the inability to appreciate spatial relationships. This presentation introduces the fundamental concepts underlying CBCT, beginning with the evolution from projection-based radiography and panoramic imaging to volumetric three-dimensional imaging.
The lecture explains how multiple two-dimensional projection images acquired during a rotational scan are mathematically reconstructed into a three-dimensional volume, which can subsequently be explored using multiplanar reconstruction (MPR). Particular emphasis is placed on orthogonal, curved and oblique MPR, demonstrating how appropriate plane selection can reveal anatomical relationships that may remain obscured on conventional views.
Beyond image acquisition and reconstruction, the presentation highlights the importance of understanding CBCT data rather than simply viewing it. Effective interpretation requires knowledge of X-ray physics, anatomical variations, artefact recognition, field of view and resolution, along with practical skills in MPR, measurement and clinical correlation. A systematic approach—initial panoramic orientation, curved-slice screening and focused oblique/MPR analysis—is proposed for navigating the CBCT volume.
Ultimately, the presentation emphasizes that CBCT does not provide an automatic diagnosis. It provides different perspectives and a richer body of evidence, while accurate interpretation remains dependent on selecting appropriate views, understanding image formation and applying clinical reasoning.
