Orofacial pain and temporomandibular disorders (TMD) pose a distinct clinical dilemma, wherein similar symptom clusters may arise from diverse and coexisting etiologies, including musculoskeletal dysfunction, neuropathic mechanisms, primary headache disorders, and psychosocial influences. This diagnostic ambiguity is further compounded by symptom overlap, variable pain behavior, and the frequent presence of central sensitization. This lecture critically examines these challenges through the lens of contemporary evidence and the Diagnostic Criteria for Temporomandibular Disorders (DC/TMD), emphasizing a structured, hypothesis-driven approach to diagnosis. Integration of Axis I physical assessment with Axis II psychosocial evaluation is highlighted as essential for accurate case stratification. The session underscores the necessity of a multidisciplinary diagnostic framework, incorporating dental, medical, physiotherapeutic, and behavioral perspectives to differentiate primary TMD from referred or comorbid pain conditions. Practical clinical pathways, recognition of red flags, and criteria for appropriate referral are discussed.
