Vital Pulp Therapy For Mature Permanent Teeth With Irreversible Pulpitis And Periapical Rarefaction - Is It Working

OBJECTIVE : 

After completing this lecture the participant should be able to have a greater understanding of new dimension in the treatment of a matured permanent teeth. 

Conventional root canal therapy for teeth with vital pulps demonstrated high success rate if performed up to the scientific standards, however,  several epidemiologic studies showed that root filled teeth were incompletely filled with technical inadequacy and apical periodontitis. 

Vital pulp therapy of cariously exposed permanent teeth remains one of the most controversial areas in dentistry. Progression of caries with pulp exposure involves local alteration related to 2 mm of coronal pulp tissue indicating that the process is localized and gradual, so if the coronally inflamed pulp tissue is removed and the remaining radicular pulp dressed with biocompatible agent, healing is expected. 

Many clinically conducted studies proved the effectiveness of vital pulp therapy for mature permanent teeth with carious exposures, irreversible pulpitis, and even teeth with PA lesions. 

Certain requirements should be satisfied for this treatment approach to be successful to be discussed in details in the lecture. 

We are living in the era of regenerative endodontics and minimally invasive endodontics , and literature is wealthy of evidence verifying validity of this alternative treatment approach.
