Accurate determination and maintenance of working length is one of the most critical factors influencing the success of root canal therapy. Loss of working length during instrumentation remains a common procedural challenge faced by clinicians and can compromise cleaning, shaping, and obturation outcomes. This complication may result from debris accumulation, canal blockages, ledge formation, apical transportation, or improper use of endodontic instruments. Failure to manage such situations can lead to persistent infection, postoperative pain, and eventual treatment failure.
This presentation aims to highlight the causes, clinical implications, and preventive strategies associated with the loss of working length in endodontic procedures. Emphasis will be placed on the role of accurate preoperative radiographic assessment, electronic apex locators, effective irrigation protocols, and proper instrumentation techniques in maintaining working length. Various management approaches—including bypassing blockages, re-establishing glide path, and use of chelating agents—will be discussed through evidence-based guidelines and case examples.
